Tibial lengthening by Ilizarov

o  corticotomy (cf. surgeon에 따라 DeBastiani’s corticotomy 시행하기도 함)

① tibial tuberosity의 직하방 crest에 2-cm incision

② 먼저 1.2-cm osteotome으로 anterior cortex를 치고

③ 5-mm osteotome으로 medial에서 lateral cortex 순으로 시행한다.

n  이유

       w   neurovascular structure 주의 – medial은 덜 위험,

       w   lateral에서 tibialis posterior 뒤쪽에 neurovascular structures

④ osteotome을 다시 insertion해서 90도로 꺾어 posterior cortex crack

⑤ distal ring compart를 externally rotating하여 확인 및 종료

(절대 internally rotate 하지 말 것, d/t peroneal nerve stretching)
