Femoral lengthening by Ilizarov

o    Lengthening의 방향 : Mechanical axis

o    Anatomical axis를 따라 시행하는 경우 문제점

①   Mechanical axis disruption

②   Knee의 medialization 되어 valgus된다.

o    anterior 에서 posterior thigh로 fixation할 때 각도

①   wire가 skin을 지나갈 때 : knee 45도 flexion

②   quadriceps muscle을 지나갈 때 : knee 90도 flexion

③   far cortex를 지나 hamstring을 지날 때 : knee full extension

④   posterior skin을 지날 때 : knee 45도 flexion

o    ASIS의 medial로 insertion하지 않도록 주의할 것. → d/t femoral nerve injury

o  lengthening 후 신연부위의 각변형 

n  knee 주변 : valgus, 후반(procurvatum)

n  hip, ankle 주변 : varus, 후반(procurvatum)

o  corticotomy

n  Site : distal ring의 proximal

n  lateral 을 먼저 시행한후 medial을 시행한다.

n  Femur bending 시켜 medial cortex를 부순다.

n  Tensor fascia lata를 transverse하게 절개하는 이유

①    Knee valgus tendency 감소

②     Lengthening을 보다 쉽게 한다.
